On your first visit to the office we request you bring a list of all medications, including dosage, you are taking. Dentistry is a medical procedure and we want to make sure we are aware of any medications you are taking, and any health issues you may have. Below we have attached a medical history form that you can fill out and either email to us or print out and bring with you when you come for your first appointment.
We welcome children and believe that the positive experience a young child has at the dentist will promote a lifetime of good dental habits and health. Many adults live with an unreasonable fear of going to the dentist because of experiences they had as a child.
